Quick scheduling of your telehelath appointment

Booking an appointment takes less than 10 minutes!

Instructions:

  1. Click the “Schedule an Appointment” tab.

  2. You will be linked to the After Hours Family Health NP PLLC telehealth site on the MEND HIPPA compliant, telehealth platform.

  3. You will be instructed to request an appointnment.

  4. You will be instructed to enter your name, contact information, reason for visit.

  5. Your appointment will be confirmed via email or text (if appropriate for telehealth) and you will be provided health history forms to complete and will be required to sign consents.

  6. You are required to submit a photo of your NY State driver license or NY State non-driver photo ID, to meet telehealth requirements in NY State.

  7. Your information will be reviewed prior to the start of your telehealth visit. During this time you will be in the virtual waiting room.

  8. Your visit will start after your medical history has been reviewed.

Medical Marijuana

The NY State Department of Health Medical Marijuana Program requires patients to be certified to obtain medical marijuana.

After Hours Family Health NP PLLC is here to help.

Schedule a Medical Marijuana Certification appointment if you have one of the many qualifying conditions. Records from your primary care provider (PCP) or therapist, documenting your diagnosis, must be provided to me before certification will be approved.

Qualifying conditions for medical marijuana certification include:

1. Opioid replacement chronic pain (as defined by 10 NYCRR §1004.2(a)(8)(xi)), pain that degrades health and functional capability as an alternative to opioid use or substance use disorder)

2. ALS

3. Cancer

4. Epilepsy

5. HIV or AIDS

6. Huntington's Disease

7. Inflammatory Bowel Disease

8. Multiple Sclerosis

9. Nausea, severe

10. Neuropathy, Chronic Pain

11. Parkinson's Disease

12. PTSD

13. Spinal cord damage to nervous tissue with objective neurological indication of intractable spacticity

If you qualify for the NYS Medical Marijuana Program, you will be certified on the NYS Medial Marijuana Data Management System (MMDMS).

You will also need to complete the patient registration at https://www.health.ny.gov/regulations/medical_marijuana/patients/

You will obtain your medication at a local medical marijuana dispensary.
